It is common enough that I've come to the conclusion that some folks think quotation marks are a way to show emphasis instead of hedging. "No" swimming: intent to emphasize the word. No "swimming": intent to hedge...what might pass as swimming is not really swimming. I’ve thought about this kind of error.
